Exiting PWI Services
You may be asked to exit the program if you:
- present danger of injury to others, property, or yourself.
- do not participate in the goals established in your IPP and/or IHC.
- do not participate in according to PWI guidelines.
- fail to abide by PWI policies and/or your employers policies.
You will also be fully supported in your exit from PWI if you:
- choose to enter another program separate from PWI.
- able to work independently without ongoing support from PWI.
Post Exit Follow Up
As PWI is concerned with the most appropriate service provision for all persons served, it is our policy to provide post exit follow-up contact. Post service follow-up information is obtained to assess the benefits and effectiveness of PWIs services. We are concerned that you will be provided the best possible service to meet your needs. Our follow-up contact will determine how well our services have benefited each individual and will empower PWI to continually improve all services provided.
If you terminate PWI services, PWI will provide a follow up contact with your Regional Center Consumer Services Coordinator (CSC). If you do not wish to have PWI provide follow-up contact, we will respect your choice and make no further contact.
Re-Entry into PWI Services
In the event you lose your job or separate from PWI for a period of time, you may be able to re-enter another PWI service. If you wish to re-enter a PWI program, please follow these steps:
- Contact your program director.
- Your PWI program director will inform you and your IDT of re-entry criteria and any available openings.
- Depending upon reasons for exit from a PWI program, re-entry may be denied.
- In the event an individual is not eligible for re-entry, referral(s) to other services will be offered.
- If it is agreed that PWI is the appropriate service to meet your needs, you, your IDT, and the PWI team will identify the best program to enter.
- The timeline for re-entry will be decided upon by you, your IDT and PWI.
